    On page 3, line 24, delete everything after "claimed" and insert "on annual premium tax returns pursuant to this Act may not exceed two million dollars in fiscal year 2017 and each year thereafter.".

    On page 4, delete lines 1 to 8, inclusive.

    On page 4, line 11, delete "Notify" and insert "Annually notify".

    On page 5, line 5, after "year" delete ";" and insert ". Contributions that are not carried forward shall be remitted to the division;".

    On page 5, line 16, delete "December" and insert "March".

            (11)    Any donation received that is not awarded a tax credit pursuant to section 2 of this Act is not subject to subdivisions (5) through (7), inclusive of this section.".

    On page 7, line 14, after "." insert "The scholarship granting organization shall report to the division the name of the insurance company, date, and amount for each contribution received from an insurance company seeking a tax credit pursuant to section 2 of this Act within thirty days of the date of the contribution.".

    On page 7, line 24, after "claimed" delete "in fiscal year 2017" and insert "on the annual premium tax return filed in 2017".
